Background
Introduction: The Occupational Therapy Health Routines (OT-HR) Screening Tool is a tool that is intended (1) to identify lifestyle patterns that could be impairing patient health, and (2) to connect patients with occupational therapy services to create more optimal habits. Occupational therapists are healthcare providers that help patients improve their performance of daily activities.
This screening tool was created to provide more comprehensive support for patients struggling with weight management. However, the screening questions are broadly about daily routines and could be applicable to other diseases, illnesses, and disabilities. Daily habits collectively have an impact on various health conditions. Therefore, it is important to have tools that can determine what lifestyle patterns could be both serving and impairing a patient’s health.
Who should use this toolkit?
This screening is intended to be used by various healthcare providers in primary care settings including occupational therapists, primary care physicians, nurses, medical technicians, etc.
What does the toolkit contain?
The Occupational Therapy Health Routines Screening Tool consists of 15 questions evaluating patient health routines, information about the purpose of the tool, screening tool usage guidelines, and incorporated scoring sections.
How should these tools be used?
The materials in this toolkit can be used:
- To identify barriers in patient lifestyle patterns, daily activities, and health behaviors.
- To guide referrals to occupational therapists for routine management.
Development of this toolkit
The Occupational Therapy Health Routines Screening Tool was developed by researchers and clinicians (Co-Investigators: Chloe Muntefering & Beth Fields) at the University of Wisconsin- Department of Kinesiology.
